About this template

Entrance Mats & Mat Service – Control Log, Slip Risk Inspection and Excel

Entrance Mats & Mat Service – Mat Register, Slip and Trip Inspection and Action Log helps Swedish housing cooperatives, landlords and property managers inventory individual entrance mats, document protruding edges, shifting and wet floors, order mat replacement or cleaning and follow up on work actually performed. It is an administrative tool for common mats and walkways at entrances – not a measurement of slip resistance or an official certificate.

Quick facts: 5 Swedish work templates, one Swedish user guide and a separate English User Guide as 7 editable DOCX + 7 searchable A4 PDFs. Also 1 XLSX with nine tabs. Total 15 customer files in a digital ZIP after manual delivery activation. The main templates are in Swedish; the English User Guide is a user manual and not a full translation.

01. Package contents

Five Swedish work documents: 01 Mat Register and Location Card; 02 Mat Inspection – Slip and Trip Inspection; 03 Non-conformance and Temporary Protective Measure; 04 Mat Replacement, Cleaning and Service Order; 05 Completion Receipt with Follow-up Inspection. In addition, 06 Swedish User Guide, 07 English User Guide and 08 Excel Tool with nine tabs are included. The seven documents are available in both Word and PDF, i.e., 14 document files plus one Excel workbook.

02. What the files contain

The templates contain fields for property, Mat ID, Location ID, actual floor surface, mat edge, slipping, moisture, dirt, walkway, service order ID, performer's receipt and separate follow-up inspection. Excel has neutral IDs, status lists, automatic duplicate warnings and administrative key figures. Ordered work is not performed work, and performed work is not automatically follow-up inspected. A visual assessment must never be marked as measured slip safety.

Who needs a mat log?

Housing cooperative: An association has several entrances with loose rental mats and needs a register where each mat is assigned a specific location and responsible service contact. Property manager: A mat edge folds over and a resident notices a trip hazard. The non-conformance is documented, an immediate protective measure is handled by the responsible person and service is ordered with its own work order. Contractor: A mat service company receipts which mats have actually been replaced; a separate responsible person follow-up inspects that the correct entrance mat is in the correct place.

Do not choose this product as the main template for outdoor winter road maintenance, full cleaning outsourcing, certified accessibility inspection, friction measurement or a technical floor inspection certificate. In a genuine acute fall risk, the walkway must be secured immediately according to the local routine, regardless of where the case is in Excel.

Format and compatibility: The DOCX files are intended for editing in a compatible word processor. The PDF files are searchable and suitable for printing but do not have digitally fillable form fields. The XLSX file requires a compatible spreadsheet program with support for formulas and data validation. Each program version has not been tested separately.

The Excel Tool – Nine Tabs and 709 Formulas

The nine tabs are Overview, Mats, Locations, Inspections, Non-conformances, Work Orders, Completions, Follow-up Inspections and Read Me. Seven registers each contain 100 empty input rows and status selections. Each register has a formula to flag DUPLICATE when the same ID appears multiple times. The overview shows unique mats and locations, performed inspections, open non-conformances, ordered and confirmed work orders as well as actually receipted completions and separate follow-up inspections.

Tested logic: Two identical Mat IDs were marked DUPLICATE, a third unique mat entry gave a unique registration and an ordered work order was counted as ordered, not as performed. Then all test entries were cleared. The customer version opens with empty registers and zero in the overview. The spreadsheet does not make a legal or technical risk assessment.

How to Use the Mat Package – Seven Steps

  1. Confirm who is responsible for each mat, the replacement routine and the entrance's actual use.
  2. Inventory entrances and mats with unique IDs and document the mat's location and floor surface.
  3. Perform an actual visual inspection of edges, shifting, wet surfaces, dirt and obstacles in the walkway. Mark anything that has not been inspected.
  4. In the event of a concrete slip or trip risk: initiate an appropriate immediate protective measure according to the organization's routines; do not wait for the order to be closed.
  5. Register a non-conformance with the actually observed condition, source, responsibility and traceable action requirement.
  6. Order mat cleaning, mat replacement or other service on your own order and collect a separate actual completion receipt.
  7. Follow-up inspect the result in reality and keep any remaining deficiencies open.

Legal Prerequisites and Risks

Slip and trip risk in the workplace: The Swedish Work Environment Authority describes falls on the same level and states that loose mats and protruding mat edges can be risk sources. AFS 2023:10, Chapter 4 deals with protection against falls in the workplace. The obligations of the regulations are directed at the responsible business operators in applicable situations; they should not be presented without an individual assessment as an identical direct control requirement for all housing cooperatives.

Floors and workplace: AFS 2023:12 contains rules that floors in the workplace must be safe against slipping and unsuitable level differences in the covered businesses.

Maintenance and accessibility: the Planning and Building Act (2010:900), especially Chapter 8, Section 14, regulates the maintenance of buildings. Boverket provides guidance on accessible entrances in building situations where the relevant requirements are applicable. A thick or loosely placed mat can create an actual passage issue that needs to be assessed locally.

Important: There is no verified general Swedish law rule in this package that requires inspection of all entrance mats once a week or that determines the same cleaning or replacement interval for all buildings. A documented "OK" is an observation, not a certificate of slip resistance.

Difference from Other Document Templates

For winter rounds and slip risk control of outdoor walkways there is another specialized package. For a broad cleaning plan and quality control there is a separate document product. Window cleaning and glass cleaning is a separate service package. This product is specifically about entrance mats' individual identity, actual condition and service measures.

Common Questions About Entrance Mats and Mat Service

Are loose entrance mats always a slip risk?

Not with an automatic yes or no. The Swedish Work Environment Authority describes that they can increase the risk, especially on floors with little surface roughness. Assess the actual mat, the floor and the passage.

Must all entrance mats be inspected every week?

There is no verified general weekly interval as a nationwide legal requirement in this product. Determine local routines based on risk, responsibility, use and agreement.

Can I use Excel to ensure accessibility?

No. Excel is an administrative workbook and can document observations, but does not issue accessibility certificates or the application of building regulations.

How do I distinguish ordered mat service from actually performed?

Work orders have their own status. Completion receipts are registered separately after work actually performed and the follow-up inspection is done in another separate tab and form.

Are the seven PDF files digitally fillable?

No. They are searchable A4 versions for reading and printing. The Word files can be edited in a compatible program.

Are all work templates translated into English?

No. Five work templates and a Swedish guide are in Swedish. A separate English User Guide shows how the Swedish material is used.
