About this template
Cat Health Diary & Veterinary Visit Log is a complete digital document package for cat owners who want to keep track of important information about identification, daily observations, vaccinations, veterinary visits, prescriptions, and expenses. Use editable Word templates for individual events, searchable PDFs for printing, and an Excel register for an overview of one or more cats.
Important distinction: This is a private documentation tool – not an official veterinary record, diagnosis, vaccination certificate, medication prescription, care guide, or registration with the Swedish Board of Agriculture. If the cat becomes ill or shows serious symptoms, contact a veterinarian.
01. Package Contents
Five Swedish templates, one Swedish user guide, and one English user guide – all as editable Word files (DOCX) and searchable, print-friendly PDFs. An Excel register with nine tabs (XLSX) is also included. Total of 15 customer files: 7 Word + 7 PDF + 1 Excel in a ZIP.
02. What the Files Contain
Cat profile and ID references, daily observations of behavior and well-being, vaccination and certificate registers, veterinary visits and treatment provided, prescription references, follow-ups, and costs. Excel distinguishes between planned and actually completed events and only calculates costs marked as actually paid.

Excel Register with Nine Tabs
The workbook contains Overview, Cats, Observations, Vaccinations, Veterinary Visits, Prescriptions, Follow-up, Costs, and Read Me. Each data log has space for up to 200 entries. Assign each cat its own Cat ID and use it in the respective registers to keep multiple cats organized.
The overview calculates the number of registered cats, documented observations, completed and planned vaccinations, completed and planned veterinary visits, prescription references, follow-ups, and actually paid amounts. Planned is never counted as completed. Quotes and estimated expenses are not counted as paid costs.
Limitations: Excel does not include automatic vaccination intervals, medication dosages, medical risk scores, clinic connections, official register integrations, or reminders. The customer version is reset and contains no fictitious record data.
How to Get Started
- Record the cat's name and a consistent Cat ID.
- Note the identification and where the Swedish Board of Agriculture's registration details can be checked.
- Record daily observations without formulating your own medical diagnoses.
- Record a vaccination as completed only when it has actually been administered and check the original certificate.
- Record care visits and refer to the veterinarian's actual records, prescriptions, and orders.
- Record follow-up visits as planned until they are completed, and track actual payments against receipts.
Cat Identification and Registration
The Swedish Board of Agriculture describes the requirement that cats should normally be identified and registered in the Swedish cat register. As a general rule, the cat should be identified and registered before it turns four months old, with special rules for changes of ownership and exceptions for cats born before January 1, 2008. This Excel workbook does not register the cat with the authorities.
Vaccination and Veterinary Care
The Swedish Veterinary Medical Institute (SVA) recommends that questions about cat vaccination be assessed together with a veterinarian. Vaccines and protection needs depend on the cat and individual circumstances. The log helps store references to vaccinations but does not calculate protection levels or future medical dates.
Note: In case of illness, injury, breathing difficulties, severely affected general condition, or uncertain medication use, contact a veterinarian. No treatments, medications, or dosages are recommended by the documents.
Frequently Asked Questions
Can I use the same Excel file for multiple cats?
Yes. Enter a unique Cat ID for each cat and use it in all register tabs.
Can I keep vaccination history?
Yes. Enter the date, vaccine according to the original certificate, clinic, and certificate reference. It becomes your own register, not an official vaccination certificate.
Does Excel calculate the next vaccination date?
No. The next date should come from the treating veterinarian and be entered manually.
Can the template be used as a veterinary record?
No. These are the cat owner's own notes. The clinic is responsible for its care documentation and original records.
Do the templates provide advice on medication or dosage?
No. They may contain references to prescriptions but do not determine preparations, dosages, or treatments.
Are the PDF files fillable?
They are searchable and print-friendly but contain no interactive PDF form fields. Use the Word files for editing.
Is all the content translated into English?
No. The five templates and Excel are in Swedish. A separate English user guide is included.
