About this template
School trips and study visits – practical document templates for risk assessment, supervision, and planning. The package is intended for Swedish schools, after-school programs, and other school authorities that need to document an activity outside regular premises. The Word files are editable, the PDF versions are searchable and printable, and Excel is used for administrative follow-up. Important: A completed template does not mean that a trip is safe, that all obligations have been met, or that the activity may be carried out without further assessment.
01. Package contents
15 files in a ZIP: five Swedish working documents, one Swedish user guide and one English User Guide, all in both Word (7 DOCX) and corresponding searchable PDF (7 PDF), plus an Excel workbook (1 XLSX) with nine tabs. The PDFs are printable versions, not interactive forms. The English guide explains the Swedish templates and does not make them suitable for foreign law.
02. What the files contain
Trip plan and decision-making basis, risk assessment with action plan, supervision and attendance checks, transport and site checks, deviation and completion report, two user guides, and an Excel register with eight key performance indicators and manual status markings.
When is the package suitable?
For a planned school trip, museum visit, study visit, or educational activity outside of school where the school authority needs clear documentation of supervision, route, staffing, checkpoints, and changed risks. Special activities such as swimming, climbing, or dangerous elements may require additional procedures, expert assessments, a responsible decision-making body, or special conditions. The template does not replace such checks.

The Excel tool's actual functions
Nine tabs: Overview, Trips, Risks, Participants, Staffing, Transport, Checks, Events, and Read Me. The overview uses eight tested formulas to count the number of registered activities, planned activities, risk items, marked verified actions, groups, staffing items, checks, and events. Data can be registered with a reference ID and selectable statuses. Excel does not provide an automatic safety assessment and deliberately keeps planned, performed, and verified separate.
How to use the templates
- Describe the activity, educational goals, participant group, location, and responsible function.
- Conduct a real risk assessment with actions, staffing, and review points.
- Check supervision, transport route, communication chain, and site risks.
- Document relevant decisions according to the school's local procedures before departure.
- Perform roll calls and handle any deviations according to the organization's emergency and reporting procedures.
- Follow up on performed actions and document their actual control.
Legal prerequisites
Supervision responsibility: The Swedish National Agency for Education describes that the school authority has supervision responsibility for students on school trips that are part of the education, based on factors including age and maturity. See the Swedish National Agency for Education's guidance on supervision responsibility. Risk assessment: the employer has its own responsibility for workplace risks and local procedures according to applicable regulations; see the Swedish Work Environment Authority. Personal data: student data must be handled with a legal basis, data minimization, and appropriate security. See the Swedish Authority for Privacy Protection's guidance. Complete class lists, sensitive data, and protected identities should not be entered into publicly shared files.
Frequently asked questions
Who is responsible for the students during the school trip?
The school authority has the ultimate supervision responsibility during an education-related trip. Practical supervision must be planned and adapted to the student group and conditions on site.
Must guardians always sign a consent form?
No, there is no general automatic rule about a specific consent form for every trip. Check the school's procedures, the nature of the activity, and when special communication or individual consent is needed.
Is it enough that Excel shows "Verified"?
No. The status is a manual administrative note, not proof that all risks have been eliminated or that the school has fulfilled all obligations.
Can the school put sensitive student data in the Excel sheet?
The workbook is designed for group ID and numbers, not sensitive personal data. Personally identifiable and protected data needs to be handled in the school's approved system according to the organization's data protection procedures.
Are fillable PDFs or English main templates included?
No. The PDF files are searchable printable versions. The Word files can be edited. The English User Guide explains the use, but the main templates are in Swedish.
