About this template
Board evaluation for non-profit organizations is a document package for non-profit boards that want to analyze their working methods and monitor improvements. The package contains editable Word templates, searchable PDF versions, and an Excel register for planned, completed, and verified actions. Important: this is internal organizational development – not a legal audit, a decision on discharge from liability, or a substitute for applicable bylaws and annual general meeting decisions.
01. Package contents
15 files in a ZIP: five Swedish working documents, a Swedish user guide and English User Guide – all in DOCX and corresponding searchable, printable PDF (14 files) – as well as an XLSX tool with nine tabs. The PDF files are not interactive forms. The Swedish working templates are not translated into English; only an English user guide is included.
02. What the files contain
The board's self-evaluation survey with working process questions, minutes for factual compilation, development plan, skills and responsibilities mapping, follow-up minutes, two guides and an Excel register. The package helps to distinguish observation, recommendation, decision, planned action, completed action and verified control.
Who is the board evaluation for?
Elected representatives, chairpersons, secretaries and non-profit boards that need a clear annual or periodic working process for internal development. The product is adapted for Swedish non-profit organizations, not for the specific corporate governance rules of limited companies. Use your own bylaws, relevant association regulations and annual general meeting decisions as a starting point.

Excel tool: nine tabs and eight compilations
The workbook contains Overview, Evaluations, Question Areas, Actions, Responsibility Map, Meetings, Competence, Follow-up and Read Me. The overview counts registered evaluations, completed evaluations, question areas, actions, manually performed versus verified actions, meetings and completed competence initiatives. Tested formulas and status selections are used for administrative tasks. Excel does not determine legal decision-making authority, liability issues, discharge from liability, lawful processing of personal data or compliance with association law.
How you work with the templates
- Review bylaws and annual general meeting decisions; define purpose, timing and participant group.
- Determine how survey responses are handled and if they can become personally identifiable even without names.
- Collect factual observations and document the compilation in the evaluation minutes.
- Make any decisions according to the association's actual mandate and minutes procedures.
- Create an action plan with responsible function and planned review.
- Document completion and verification separately and follow up on open questions.
Legal and practical prerequisites
Bylaws and decisions: A non-profit board works in relation to applicable bylaws and the annual general meeting's decisions. See Swedish Tax Agency's guidance on non-profit organizations and the board's administration. Personal data: even a small internal survey can indirectly identify participants; limit data and access based on GDPR. See IMY's guidance for associations. An evaluation is not an auditor's review, and improved working methods do not automatically grant discharge from liability.
Frequently asked questions
Must a non-profit organization conduct a board evaluation every year?
There is no general legal requirement for this specific evaluation form for all non-profit organizations. Check bylaws, association regulations and decisions for your specific organization.
Is the board's self-evaluation the same as an auditor's report?
No. The self-evaluation examines the board's working methods. The auditor's financial and administrative review is a separate task. See Auditor's Report & Audit Documentation.
Can the survey be anonymous?
Yes, but anonymity may be limited in a small board where answers and free text make it possible to identify individuals. Conduct a special data protection assessment.
Does a completed action plan give the board the right to make new decisions?
No. An improvement plan does not in itself provide legal authority. Decisions must be made according to the bylaws and relevant delegation of authority.
Are English board templates and fillable PDF files included?
No. The main templates are in Swedish, and an English-language user guide is included. The PDF files are searchable and printable but lack interactive form fields.
