About this template

Appointing a Data Protection Officer (DPO) under GDPR – a practical document package for Swedish organisations that need to appoint, document and monitor a DPO function. The templates help management, data controllers and compliance functions organise decisions, resources, independence, contact details and ongoing reporting. The delivery includes editable Word files, searchable PDF files and Excel. Important: the package does not automatically determine whether your organisation is required to appoint a DPO and does not replace the IMY notification.

01. Package contents

15 files in a ZIP: five standalone Swedish working documents, a Swedish user guide and an English-language user guide – all in DOCX and searchable PDF – plus an XLSX workbook with nine tabs. Word files can be edited; PDFs are read-only and printable versions, not interactive forms. The English User Guide explains the Swedish documents and is not an English contractual version.

02. What the files contain

Decision template for appointing a DPO, assignment and resource plan, conflict of interest assessment, publication and IMY notification log, annual plan and reporting protocol, two guides plus a register for documented follow-up of advice, controls and decisions. The files distinguish planned steps from actually performed and verified actions.

When is the template package suitable?

For data controllers and data processors in Sweden who need to prepare a decision basis for a mandatory or voluntary DPO appointment and organise the work in accordance with Articles 37–39 of the GDPR. The documents are not an external DPO agreement, a general privacy policy or a regulatory decision. A separate substantive assessment of any national special rules, professional qualifications, independence and conflicts of interest is required.

Excel tool – nine tabs

The workbook contains Overview, Organisation, Annual Plan, Advice, Controls, DPIA, IMY and Contact, Decisions and Read Me. An overview counts registered activities and advice as well as manually status-marked completions and controls. The formulas have been function-tested. The status lists make it easier to organise the work, but neither a filled-in row, a completed step nor the Excel status itself constitutes legal compliance. No macro is required.

How the templates are used

  1. Document whether the appointment is mandatory under GDPR Article 37 or voluntary.
  2. Assess whether the candidate meets the competence requirements, is independent and has no unresolved conflicts of interest.
  3. Make a proper decision and document the assignment, reporting line and resources.
  4. Publish contact details and notify IMY via the correct channel; document acknowledgement separately.
  5. Establish an annual plan and use Excel for actual follow-up of advice, controls and decisions.

Legal requirements

GDPR Article 37 regulates, among other things, when a data protection officer should be appointed, what professional qualifications are required, and the publication and notification of contact details to the supervisory authority. Article 38 sets requirements for the DPO's position, early involvement, resources, independence, reporting and absence of conflicts of interest. Article 39 describes the DPO's core tasks, including advice and monitoring. A voluntarily formally appointed DPO is also subject to these requirements. The assessments must be based on actual operations and an actual mandate. The package contains no regulatory form and is not approved by IMY.

Frequently asked questions

Must all companies appoint a data protection officer?

No. The requirement depends on the processing and operations covered by Article 37.1 and other applicable law. Even if the appointment is voluntary, a formally appointed DPO function needs to follow relevant rules.

Is the decision template the same as the notification to IMY?

No. It is an internal documentation basis. Publication and notification of the DPO's contact details must be carried out separately in practice.

Can a manager also be a DPO?

It depends on the manager's actual roles. Assignments that involve the person determining the purposes and means of personal data processing can create an impermissible conflict of interest. A concrete assessment is required.

Do I get English master templates?

No. The five working documents are in Swedish. An English User Guide explains how the Swedish documents are used; the product is intended for GDPR/Sweden.

Are the PDF files fillable?

The PDF files are searchable and printable, but not interactive forms. Use the Word versions for editing.

Related templates

In the event of an actual data protection incident, other working documents are needed, for example template packages for personal data incidents. For general data protection administration, you can also see Mallbutikens GDPR and data protection templates. The DPO package does not replace a data processing agreement, a DPIA or an incident procedure.

Sources, version and review

Legal framework: Data Protection Regulation (EU) 2016/679, Articles 37–39; IMY's guidance on DPO. Product version 1.0. AI-assisted legal source check performed 1 November 2026. No independent external legal review of this product version is documented. The digital ZIP is delivered through the store's regular solution only after the product has been published and the delivery file linked.
