About this template

Dog Health Diary & Veterinary Visit Log is an editable digital document package for dog owners who want to collect important information about their dog's medical history, vaccinations, veterinary visits, treatments, observed symptoms, weight, and costs in one place.

Scope: This is a private administrative aid—not a medical record created by a veterinarian, a medical prescription, a vaccination certificate, an EU pet passport, or an official ownership register. Treatment decisions should always be made in consultation with a veterinarian.

01. Package Contents

5 Swedish working templates, 1 Swedish user guide, and 1 English User Guide. All seven documents are provided in both editable Word (DOCX) and searchable PDF formats. The package also includes 1 Excel workbook (XLSX) with nine tabs. Total of 15 files in a ZIP: 7 Word + 7 PDF + 1 Excel.

02. What the Files Contain

Dog profile, veterinary visit log, vaccination history, treatment and medication notes, symptom/weight/cost log, guides, and a comprehensive Excel workbook with a formula-driven overview.

When Is a Dog Health Diary Useful?

When you need to keep track of which clinics the dog has visited, preserve references to original certificates, view previous notes before follow-up appointments, and understand how healthcare costs are distributed. Always keep the veterinarian's actual medical records, vaccination certificates, prescriptions, receipts, and insurance statements separately.

What Can Excel Do?

The workbook includes the Overview, Dog Profile, Veterinary Visit, Vaccinations, Treatments, Symptoms, Weight, Costs, and Read Me tabs. It summarizes the number of registered dogs and notes, number of weight measurements, administratively completed visits, and manually entered costs in SEK. The formulas have been checked using fictitious test data. The customer version is delivered without test entries. Up to 200 registration rows per data tab are supported.

Excel provides no medical diagnosis, dosage, risk assessment, or automatic calculation of vaccination intervals and does not send notifications.

How to Get Started

  1. Create a working copy and fill in the dog's details.
  2. Assign the dog a consistent Dog ID in Excel if you manage multiple dogs.
  3. Keep copies of original documents from the veterinarian and insurance company.
  4. Record visits and vaccinations with the date, clinic, and certificate reference.
  5. Only enter treatment information that comes from a current veterinary prescription.
  6. Record observations objectively and follow up with a veterinarian if concerned.
  7. Check the Excel overview against the actual records.

Important Sources and Limitations

The Swedish Veterinary Medical Institute (SVA) advises dog owners to contact a veterinarian regarding vaccination questions and if the animal is ill. Read SVA: Dog Vaccination.

All dogs must be marked and registered in the Swedish Board of Agriculture's official register. This private health diary does not register the dog with the authority. Read Swedish Board of Agriculture: Mark and Register Dogs.

Frequently Asked Questions

Is the health diary an official veterinary record?

No. It is for your own notes and links to official clinic documents.

Does the template create vaccination certificates or dog passports?

No. It helps you document information from actual certificates, but does not replace them.

Can Excel calculate the next vaccination or medication dose?

No. Only the treating veterinarian can provide dog-specific advice on needs, intervals, and prescriptions.

Can I use the same Excel workbook for multiple dogs?

Yes, enter your own Dog ID in the registration tabs and check that each entry refers to the correct dog.

Are the PDF files fillable?

The PDF is searchable and print-friendly but contains no interactive form fields. The Word files are editable.

Are all documents in English?

No. The five working templates and Excel workbook are in Swedish. A separate English user guide is included.
