About this template
IT Change Request & Change Management is a complete digital template package for businesses and IT organizations that want to manage changes to systems and services in a more structured way. Document Change Request (RFC), impact, risk assessment, testing, approval, deployment, rollback plan, and post-implementation review in editable Word files and a trackable Excel register. The package is delivered as a ZIP file with 15 customer files.
01. Package Contents
Five Swedish working documents + Swedish and English user guides in both Word and searchable PDF, plus a separate Excel workbook. Total 7 DOCX, 7 PDF, and 1 XLSX (15 files). The working files are in Swedish; the English User Guide provides instructions in English. Delivery: digital ZIP for download after purchase.
02. What the Files Contain
Change request with acceptance criteria, impact and security assessment, traceable decision-making documentation, testing and go/no-go, rollback plan, and verified post-implementation review. The Excel workbook has nine tabs with registers, risk scores, status selections, a decision log, and an overview. It distinguishes between requested, approved, implemented, and actually verified changes.
What is a Change Request (RFC)?
A Request for Change documents what needs to be changed in an IT system, why, which dependencies are affected, and what decision is required before work begins. Structured change management aims to reduce unnecessary operational disruptions and create traceability around responsibility, testing, decisions, and rollback. The package is a practical aid, not a technical system that implements or approves changes.

Excel Tool with Nine Tabs
The workbook contains Overview, Changes, Risks, Tests, Approvals, Deployment, Rollback, Post-Implementation Review, and Read Me. The register tabs are linked by their own change IDs. Risk scores can be calculated as probability multiplied by impact on an internal 1–5 scale when both values are specified. The dashboard only counts registered IDs, not empty preformatted rows. The user must assess the status and final approvals themselves.
How to Use the Package
- Register a request and a unique change ID.
- Describe the current and desired state, dependencies, and acceptance criteria.
- Assess risks, operational impact, information security, and any personal data flow.
- Prepare testing, communication, and a rollback plan.
- Document the decision by a person with actual authority according to the organization's procedures.
- Implement and log only the approved change.
- Check the outcome, address outstanding points, and document the issue closure.
When Is This Package Suitable?
For planned system updates, configuration changes, deployment, and follow-up on changes in services or IT environments. It complements a backup and restore testing template, but does not replace technical testing, incident plans, or an actual system management solution. The forms are not contractual changes between parties, nor are they templates for statutory reporting.
Frequently Asked Questions
What do RFC and CAB mean?
RFC stands for Request for Change – a change request. CAB (Change Advisory Board) is one possible organizational form for reviewing certain changes. Who actually has decision-making authority depends on the organization's own rules and any agreements, not the template itself.
Can the package be used for urgent IT changes?
Yes, with a locally adapted decision-making and documentation process. Urgent does not automatically mean an exemption from security requirements or authorities. Those responsible must determine what applies to a genuine urgent change.
Is the Excel register an automatic approval system?
No. Excel compiles registered data and can calculate a simple internal risk score. It does not send approvals, control permissions, or verify operational reliability.
What is the difference between implemented and verified?
Implemented describes that a work step has been completed. Verified means that there is a test result according to a predetermined criterion. Approved and closed require separate decisions according to the business process.
Are the PDF files fillable?
The PDF files are searchable and can be printed, but do not contain interactive PDF forms. To edit or fill them in, use the Word versions and the Excel register.
Do the templates meet NIS2, DORA, ISO, or ITIL?
No, no automatic requirement fulfillment or certification is included. Which rules apply depend on the business, sector, agreements, and actual implementation. For example, special regulations on the information security of government agencies may not be assumed to apply to all companies.
What is included in English?
An English User Guide in both Word and PDF. The five main Swedish templates are not included translated into English.
