About this template

A3 Problem Solving & PDCA is a digital template package for improvement work. Create a clear A3 report, formulate a measurable problem, test changes, and document what actually worked. For companies, associations, and improvement teams working with processes and quality. Editable Word (DOCX), searchable PDF, and Excel (XLSX). Note: this is a voluntary methodology template, not a government form, safety risk assessment, or ISO certification.

01. Package Contents

5 Swedish working documents in both editable Word and searchable PDF, a Swedish user guide and an English User Guide in Word/PDF, plus 1 Excel workbook. Total of 15 customer files: 7 DOCX + 7 PDF + 1 XLSX. Delivered digitally as a ZIP. The main A3 report has an information page and an actual A3 landscape page; the other documents are A4.

02. What the Files Contain

A3 report, problem definition and current state, testing of cause hypotheses, PDCA test cards, and action and effect control. The guides show the workflow and limitations. Excel has nine tabs with separate statuses for planned, completed, and verified effect, as well as formula-driven change calculation.

What is an A3 report and PDCA template used for?

An A3 report helps a team describe the problem, current state, desired result, possible causes, tested interventions, and follow-up on a single overview page. PDCA (Plan–Do–Check–Act) provides a process for testing a change and assessing the outcome. The template is a tool for learning and decision-making, not an automatic check that improvement or regulatory compliance has actually been achieved.

When is the package suitable – and when is something else needed?

Choose A3 for a defined improvement project where you can describe the current state and objective, work iteratively, and measure effects. For a specific quality non-conformance with customer requirements for 8D, an 8D report package may be more suitable. For broad process mapping, there is a separate process mapping package. Do not use the A3 template instead of statutory workplace risk assessment, regulated incident reporting, regulatory product recall, or prescribed operational documents.

Excel Tool – Nine Real Working Tabs

The workbook contains Overview, Cases, Current State, Hypotheses, PDCA Tests, Actions, Measurements, Decisions, and Read Me. In the Measurements tab, absolute difference is calculated as the after-value minus the before-value when both values are numbers; percentage change is compared with the absolute value of the before-value. If the before-value is zero, “Not calculable” is displayed for the percentage. A positive difference does not automatically mean improvement: the desired direction and effect must be assessed manually. Pre-filled rows, filters, and status selections make it easier to separate administrative steps from actually verified effect.

How the Template Package is Used

  1. Register the improvement case with a unique A3 ID.
  2. Define the problem with observations, measures, units, data source, and period.
  3. Note several possible causes and test the hypotheses against evidence.
  4. Plan a defined PDCA test with necessary risk and approval checks.
  5. Record actual activity separately from planned activity.
  6. Measure the after-value and assess whether the comparison is fair.
  7. Document decisions, responsibility, follow-up, and any standardization.

Frequently Asked Questions

What is an A3 report?

A visual summary of an improvement project, often on a landscape A3 page. It shows the problem, current state, possible causes, interventions, results, and next check. It is a voluntary working method.

What is the difference between A3, PDCA, and 8D?

A3 is primarily a summary structure and a way to communicate the improvement work. PDCA describes an iterative working method: plan, do, check, and act. 8D is another method often used for systematic problem investigation and corrective actions.

Does Excel Make Automatic Quality Assessments?

No. Excel calculates numerical difference and percentage change when values are present. It does not determine whether an action is approved, whether the desired result has been achieved, or whether a legal requirement is met.

Is the A3 PDF Fillable?

The PDF files are searchable and printable. Edit the Word version to fill in the information. Interactive PDF forms are not included.

Are the Templates ISO- or Government-Approved?

No. Mallbutik documents are independently designed voluntary working tools. They do not replace requirements in government regulations, special agreements, or the organization's existing routines.

What Does English User Guide Mean?

The English guide explains the Swedish working documents. English translations of the main templates are not included.
