Parental Consent / Travel Consent for a Minor 2026/2027 – Swedish & English Word/PDF
This is a professional and clear travel consent form for a minor for situations where a minor child is traveling alone, with only one legal guardian, or with another adult. The package contains both a Swedish version and an English Travel Consent for a Minor, plus a detailed user guide.
The template package was reviewed against current Swedish regulations and authority recommendations on October 4, 2026, and is prepared for use during 2026/2027. The Swedish Police do not have a general Swedish standard form for travel consent but recommend some form of certificate from legal guardians when a child travels without all legal guardians accompanying them. At the same time, the destination country, transit country, and carrier may have their own requirements.
Delivery: 3 documents in both Word (DOCX) and PDF – a total of 6 files and 16 A4 pages. The product is delivered digitally. No physical goods are sent.

When can a travel consent be relevant?
The document is designed for the most common situations where additional proof of legal guardians' approval may be requested:
- the child is traveling alone,
- the child is traveling with only one of two legal guardians,
- the child is traveling with a grandparent, relative, friend, or other adult,
- the child is traveling in an organized group or in a situation where the carrier wants to be able to verify consent.
The template does not replace the child's passport, national ID card, visa, or any other mandatory travel document.
No universal Swedish standard form
The Swedish Police state that Sweden does not have a general standard form for permission from legal guardians when traveling to and from Sweden. The police still recommend some form of certificate when children travel without all legal guardians accompanying them and emphasize that names and contact details should be clearly stated so that the information can be verified.
This template package is therefore designed as a practical supporting document and not as an authority-issued permit.
Always check destination, transit country, and carrier
Rules vary between countries. A country may require a special form, witnessing, a notary public, legalization, an apostille, a copy of a birth certificate, or other documents. Even a country that is only passed through during a layover may have its own requirements.
Airlines, ferry companies, train operators, or other carriers may also have their own age limits and their own forms, especially for children traveling without adult supervision.
The user guide therefore contains a special four-level check before the trip:
- Sweden,
- the destination country,
- any transit countries,
- the carrier.
Swedish and English version
The package contains two separate documents with the same basic structure:
- Parental Consent / Travel Consent for a Minor in Swedish.
- Travel Consent for a Minor in English.
For international travel, it is often practical to carry both versions together. The English version makes it easier for border control, airlines, hotels, and other stakeholders to understand the document without an on-site translation.
Child's information
The template contains clear fields for:
- full name,
- personal identity number or date of birth,
- citizenship,
- passport or national ID number,
- validity period of the travel document,
- registered address,
- phone number where relevant.
Guardianship status documented
The document has a specific section where the user can specify if the child has:
- joint custody,
- sole custody,
- other custody status.
If a parent has sole custody, it may be relevant to bring official documentation proving this, depending on the recipient country's requirements.
Information about the legal guardians
For each legal guardian, there are fields for, among other things:
- full name,
- personal identity number or date of birth,
- address,
- phone number with country code,
- email address,
- passport/ID details when needed.
This makes it easier for border control or a carrier to verify the consent if questions arise.
Accompanying adult
When the child travels with someone other than the legal guardian, the document can specify:
- the adult's full name,
- relationship to the child,
- date of birth,
- citizenship,
- passport number,
- phone number and email.
It is also possible to indicate that the child is traveling alone.
Trip clearly specified
A central part of the consent is that the trip is described as concretely as possible. The template therefore has fields for:
- country of departure,
- destination or destination countries,
- transit countries,
- date of departure,
- planned date of return,
- airline or other carrier,
- booking or flight number,
- main residential address abroad,
- phone number to accommodation or host.
There is also a separate table for multiple segments or additional travel plans.
The consent itself
The legal guardian or guardians certify that the child may carry out the specified trip according to the information in the document. When the child travels with another adult, the document can also state that this person may assist the child with practical travel issues and contact with carriers, accommodation, and authorities to the extent permitted.
The template clearly explains that the travel consent is not a general power of attorney and does not automatically transfer custodial rights or decision-making rights.
Emergency medical care and emergencies – optional information
There is a separate optional section where the legal guardians can provide practical information that may be relevant in an emergency, such as:
- allergies,
- important medications,
- insurance company and policy number,
- European Health Insurance Card where relevant,
- extra contact person.
The section is deliberately limited and does not constitute a guarantee that a foreign hospital will accept the document as a medical power of attorney.
Attachments that can strengthen documentation
The travel country's rules determine what is needed, but the user guide goes through common additions such as:
- copy of the child's passport/ID page,
- copy of the non-accompanying legal guardian's ID document,
- civil registration certificate, birth certificate, or equivalent document showing the family relationship,
- documentation of sole custody,
- travel booking and accommodation address,
- insurance details,
- country-specific official form when required.
Witnesses, notary public, and apostille
Sweden does not have a general requirement that this type of travel consent must be witnessed or notarized for all trips. Another country or a carrier can, however, set specific formal requirements.
The template therefore contains optional fields for two witnesses as well as a notary public/other verification. Use them only when they are actually needed according to the recipient's rules.
Children traveling alone
For children traveling without adult company, the carrier's rules must be checked specifically. Airlines may have their own age limits, requirements for so-called Unaccompanied Minor service, and separate forms.
The travel consent complements such carrier documents but does not replace them.
Group travel, school trips, and association trips
For organized school, association, or group trips, special rules may apply. The template can serve as support for individual consent, but the organizer should simultaneously check if there are special group lists, authority forms, or carrier requirements.
Detailed user guide
The separate guide helps the user to use the document correctly and contains, among other things:
- what the template is and is not,
- when travel consent is particularly relevant,
- the four-level check before the trip,
- checking the child's travel document,
- how the Swedish and English versions are filled out,
- joint versus sole custody,
- which attachments can strengthen the certificate,
- witnesses, notary public, and apostille,
- children traveling alone,
- group travel and school trips,
- emergency medical care and document limitations,
- final check before signature.
Reviewed for 2026/2027
The template package was reviewed on October 4, 2026, against current Swedish authority recommendations and European travel information. The Police Authority's information on traveling with children states that rules vary between countries and that a certificate from legal guardians is recommended when children travel without all legal guardians.
The EU's official Your Europe information also emphasizes that there are no common EU rules on such permits and that each country decides which documents are required. Transit countries and airlines may have their own requirements.
The designation 2026/2027 therefore means that the template package has been reviewed against the known legal and authority situation at the date of review. The traveler must still make a current check before each individual trip.
